We are not longer enrolling pets in wellness plans. However, if you are in an auto-renewing plan for your pet, that plan will be continued as long as you maintain continuous enrollment.

You can update your plan purchased before 12/1/22 anytime by filling out this form or speaking to us in the clinic. These plans auto-renew annually with the card on file and will remain in effect until canceled or unpaid. If you would like to cancel your wellness plan, simply let us know. If your pet becomes deceased or you move more than 30 miles away from a Pet’s Favorite Vet location, a prorated remainder of the fee will be refunded. Wellness Plans are not transferable or refundable in the event of the rehoming of a pet.
